requests to be a member of Los Tejanos Del Sur. By signing below, the undersigned individual understands that participation with Los Tejanos Del Sur may include participation in a recreational and/or physical activity, as well as travel, the undersigned hereby assumes the risk of injury inherent in such physical activity or travel to and from any event and waives any claim for damages of any kind, including serious injury, against the Muay Thai Alliance of Texas (MTAT), Los Tejanos Del Sur, RGV Ocelots FC, and the Houston Texans and all parties respective sponsors and/or personnel for any injuries that may be sustained during such activity, event, or travel.
In recognition of any media coverage alotted to Los Tejanos Del Sur thereto, the undersigned grants Los Tejanos Del Sur, the MTAT, and/or RGV Ocelots FC, and and its licensees her permission and authority to use his/her name, voice, picture, and likeness in connection with any and all publications, media, broadcasts, and promotional photographs including but not limited to event pictures, advertisements, announcements, newsletters and any other commercial items with no compensation to applicant. Any such use of the aforementioned shall be for the purpose of promoting this supporters' club.
I affirm that I am of sound mind and body to register, and that I am the person registering with Los Tejanos Del Sur. I further affirm that all of the information provided to this organization is true and correct to the best of my knowledge and ability.
I further affirm that I will follow all rules and policies as set out by Los Tejanos Del Sur when representing the club at any event and understand that my failure to do so constitutes good cause for termination of my membership.
